Symptoms improvement and lung function benefits of fixed triple therapy with beclomethasone/formoterol/glycopyrronium inhalation powder in patients with moderate-to-severe COPD: outcomes of the Bulgarian arm of the RESPONSE study.

Petkov Rosen E RE, Kenderov Andrey N AN, Rodopska Neli L NL, Mekov Evgeni V EV et al.

The fixed triple beclomethasone dipropionate/formoterol fumarate/glycopyrronium (BDP/FF/GB) therapy is available in both pressurized metered-dose inhaler (pMDI) and dry powder inhaler (DPI) formulations for chronic obstructive pulmonary disease (COPD) treatment. While the pMDI form has been extensively investigated in pivotal trials and real-world studies, real-world data on the DPI (88/5/9 μg) formulation remain limited. The RESPONSE study assessed the real-world effectiveness of BDP/FF/GB inhalation powder in a cohort of moderate-to-severe COPD patients uncontrolled with dual combinations. This non-interventional, prospective, multicenter study was conducted across 21 Bulgarian sites. COPD patients (≥35 years) with persistent symptoms [COPD Assessment Tool (CAT) ≥10 and/or modified Medical Research Council (mMRC) ≥2] and moderate or severe pulmonary obstruction [30%≤ forced expiratory volume in 1 second as a percentage of predicted value (FEV1%) <80%] despite dual therapies were switched to BDP/FF/GB DPI and followed for 6 months. The primary endpoint was improvement in CAT scores. Secondary endpoints included lung function, treatment adherence, dyspnea and the percentage of patients achieving CAT score reduction ≥2 from baseline. A total of 363 patients completed the study. Mean CAT scores improved significantly from 23.0 at baseline to 16.9 (P<0.001), regardless of prior dual therapy or inhaler type. Mean (± SD) FEV1% increased from 47.6%±11.9% to 53.2%±15.75% (P<0.001). Treatment adherence and dyspnea also showed significant improvement. Notably, 87.9% of participants achieved a CAT score reduction ≥2 points. The RESPONSE study showed that switching to BDP/FF/GB DPI was associated with clinically meaningful improvements in symptoms, lung function, and adherence in COPD patients inadequately controlled on dual therapy, supporting its role as a practical escalation option in routine care.

Efficacy of Calcipotriol/Betamethasone Ointment in the Management of Pediatric Alopecia Areata.

Won Yumi Y, Lee Jungsoo J, Kim Hoonsoo H, Ko Hyunchang H et al.

Topical corticosteroids are acknowledged as the first-line treatment for pediatric alopecia areata. There are cases that are recalcitrant to topical corticosteroid application. The objective was to evaluate the efficacy of calcipotriol/betamethasone ointment in pediatric alopecia areata. In a 12-week retrospective study, 40 patients under 10 years old with alopecia areata were included. The patients were treated with a topical ointment combining 0.005% calcipotriol and betamethasone dipropionate once daily. The efficacy was monitored through changes in Severity of Alopecia Tool (SALT) scores. The 40 patients, with a mean age of 4.1±2.1 years, were categorized by their SALT scores into two groups: those above and below 50. Patients with SALT scores over 50 showed hair regrowth in 7.44±4.44 weeks, while those with SALT scores under 50 showed regrowth in 5.71±2.58 weeks. After 12 weeks, 80.0% of the patients showed hair regrowth and 20.0% of patients showed no response or worsening. Only minor adverse events were documented during follow-up. Calcipotriol/betamethasone ointment may represent a useful topical option for pediatric alopecia areata.

Early versus late escalation to extrafine BDP/FF/GB single-inhaler triple therapy in COPD: a real-world subgroup analysis by disease duration since diagnosis.

Rogliani Paola P, Brusselle Guy G, Stolz Daiana D, Piraino Alessio A et al.

The optimal timing for escalation to triple therapy in chronic obstructive pulmonary disease (COPD) remains undefined, often resulting in delayed treatment. Consequently, evidence on the effectiveness of early initiation is limited. This subgroup analysis used pooled patient-level data from six observational studies to evaluate the real-world effectiveness of beclomethasone dipropionate/formoterol fumarate/glycopyrronium bromide single-inhaler triple therapy (SITT) in COPD according to disease duration. Patients were stratified into three subgroups: ≤48 months (short), 49-96 months (intermediate), and ≥97 months (long-standing). Outcomes assessed at 3, 6, and 12 months post-SITT initiation included mean number of exacerbations, proportion of patients experiencing ≥1 exacerbation, and proportion achieving disease stability and control. Mean number of exacerbations decreased from the year prior to SITT initiation at all post-treatment time points across all subgroups, with the largest reductions observed in short disease patients, particularly at 12 months. Similarly, the proportion of patients experiencing ≥1 exacerbation decreased in all subgroups, with the lowest proportions observed in short disease patients. No differences between subgroups were observed at 3 and 6 months in the proportion of patients achieving disease stability and control; conversely, at 12 months the short disease subgroup showed significantly higher rates of disease stability and control compared with longer disease duration subgroups, suggesting that earlier SITT initiation may provide more sustained benefits. Early escalation to SITT in COPD may result in more durable sustained clinical benefits compared with later intervention, reducing exacerbations and maintaining disease stability and control up to 12 months.

A New Restrictive Composite Endpoint to Assess Best Response in Patients with Mild-to-Moderate Psoriasis Treated with CAL/BDP PAD Cream: A Case Series.

Pinter Andreas A, Galván Jordi J, Freischläger Frank F

In the assessment of a psoriasis treatment, traditional endpoints evaluated independently may not fully capture the multidimensional nature of treatment response. Best response to calcipotriol/betamethasone dipropionate (CAL/BDP) cream based on polyaphron dispersion (PAD) technology has recently been defined as a restrictive composite endpoint comprising two efficacy outcomes (Physician's Global Assessment [PGA] 0-1 and modified Psoriasis Area and Severity Index [mPASI] <2) and one quality-of-life outcome (Dermatology Life Quality Index [DLQI] 0-1). This retrospective case series aims to provide further insights into the demographics and outcomes of patients achieving best response to CAL/BDP PAD cream based on this newly defined composite endpoint. From the 213 patients with mild-to-moderate psoriasis who received CAL/BDP PAD cream in the European MC2-01-C7 phase 3 clinical trial (NCT03802344), 7 patients who achieved best response at week 4 and/or week 8 and had available clinical photographs were evaluated. The mean patient age was 55.0 years (range: 34-68), with a mean psoriasis duration of 19.9 years (range: 6.9-29.1). Results support the efficacy of CAL/BDP PAD cream in improving both clinical signs and quality of life. Despite existing variability in key baseline characteristics and long disease duration, all included patients met the novel restrictive composite endpoint after 8 weeks, which reflects complete/almost complete clearance of psoriasis and no impact on patient quality of life.

Rapid Improvement of Refractory Pruritus in Extragenital and Perianal Lichen Sclerosus With Nemolizumab: A Case Report.

Frucht Corey S CS, Ragsdale Bruce D BD

Lichen sclerosus is a chronic inflammatory dermatosis that most commonly affects anogenital skin but may also involve extragenital sites. Pruritus can be severe and may persist despite standard anti-inflammatory therapy. Nemolizumab is a monoclonal antibody targeting interleukin-31 receptor alpha, a pathway involved in chronic itch. We report a 68-year-old woman with biopsy-proven lichen sclerosus involving the upper back, chest, shoulders, and perianal area, with several truncal lesions localizing to surgical scars from prior post-bariatric body-contouring surgery. She reported remote genital involvement several years earlier. At the eight-week follow-up, shiny white plaques involved the perianal area, but no vulvar involvement was identified. The patient had severe pruritus affecting sleep and daytime function. Prior treatments included betamethasone dipropionate 0.05% cream, intralesional triamcinolone acetonide, and office-based narrowband ultraviolet B phototherapy, the latter producing remission from 2021 until early 2026 before recurrence. Because of recurrent severe pruritus, nemolizumab was started off-label with a 60 mg subcutaneous loading dose followed by 30 mg every four weeks. No other topical, systemic, or phototherapy treatment was used after nemolizumab initiation through the first eight weeks. At four-week follow-up, patient-reported pruritus improved from 8/10 to 0/10. At eight-week follow-up, pruritus remained markedly improved, although she reported a transient flare of burning in the perianal area several weeks earlier that had since markedly improved. No adverse effects occurred. Examination continued to show shiny white plaques, including perianal involvement, without clear morphologic remission. A short pulse of clobetasol 0.05% ointment was initiated for perianal disease at the eight-week visit while nemolizumab was continued. This case suggests that nemolizumab may provide rapid symptomatic relief of refractory pruritus associated with lichen sclerosus, even without early morphologic resolution.

Development and evaluation of microemulsion foam formulation for topical delivery of betamethasone dipropionate.

Zhang Furong F, Ju Xiaoli X, He Meilin M, Sun Shuai S et al.

Betamethasone dipropionate (BDP) is widely recognized as a therapeutic agent for psoriasis. However, the current topical formulations exhibit certain limitations, including significant systemic absorption and substantial residue on the skin surface post-application. Thus, this study aimed to develop a novel microemulsion (ME) foam system, which can enhance the targeting and bioavailability of BDP at the diseased skin. Through the pseudo-ternary diagrams, it was determined that when the ratio of surfactant to cosurfactant was 6:1, the ME region reached its maximum. The optimized formulation was composed of 0.064% BDP, medium-chain triglyceride (oil phase), Labrasol/polyglyceryl-3 dioleate NF (surfactant/co-surfactant), and water (aqueous phase). In vitro permeation studies on normal and exfoliated porcine skin, the ME foam could significantly increase the R/Q (ratio of intradermal retention to permeation) of BDP compared to the two commercial formulations (Diansong cream and Xamiol gel). The in vivo studies on rats showed that compared to the commercially cream, the ME foam significantly increased the 24-hour intradermal retention of the drug, and the Tmax of plasma pharmacokinetics was also longer. This further demonstrated the intradermal targeting and sustained-release properties of the ME foam. Overall, the ME foam developed in this study is a promising, safe and effective drug vehicle for the topical delivery of BDP.
