Analysis of 92 hospitalized cases of non-neonatal tetanus in Guizhou Province, China.

Li Jiao J, Zhang Dandan D, Zhang Ruizhi R, Zhang Leibing L et al.

Tetanus has a high mortality rate and remains a major public health threat in low- and middle-income countries. Although neonatal tetanus has been eliminated in China, the burden of non-neonatal tetanus is still heavy. This study analyzed the case characteristics, outcomes, and hospitalization expenses of non-neonatal tetanus inpatients in Guizhou Province, China, and provided a basis for reducing the disease burden. One tertiary medical institution was randomly selected from each of the nine cities (states) in the province, and the data of non-neonatal tetanus hospitalized cases from 2021 to 2025 were collected. Descriptive epidemiological methods, the simple seasonal index method, and multiple linear regression were used in the analysis. A total of 92 cases were included. Males accounted for 65.22% (60/92); farmers accounted for 86.96% (80/92); the median age was 60 (55-69) years; the incubation period ranged from 1 to 27 days; and cases peaked in summer. 95.65% (88/92) were not vaccinated with tetanus toxoid-containing vaccine (TTCV), and only 21.74% (20/92) received standardized treatment within 24 h after injury. The case fatality rate was 15.22% (14/92). None of the non-survivors were inoculated with TTCV, no standardized treatment was performed within 24 h after injury, the severity was severe or above, and the family members gave up treatment on their own. The median hospitalization cost was 55,724 yuan (equivalent to 8,359 US Dollars); the highest was 239,365 yuan (equivalent to 35,905 US Dollars); and the number of hospitalization days was the main influencing factor (t = 5.751, p < 0.001). Non-neonatal tetanus in Guizhou Province was mainly in middle-aged and older male farmers, with a high incidence in summer, and most of them were not vaccinated with TTCV. The non-survivors were not vaccinated with TTCV and did not receive standard treatment within 24 h after injury. The disease had a high case fatality rate and was a serious economic burden. It is recommended to strengthen TTCV vaccination and standardized post-exposure disposal in high-risk groups, enhance medical financial support to prevent abandonment of treatment for economic reasons, and ensure that patients receive timely and comprehensive clinical care.

Oncolytic virus VT1092M synergizes with immune checkpoint inhibitors against intraperitoneal metastases of colorectal cancer.

Hu Zongfeng Z, Yuan Xinru X, Li Shuzhen S, Liu Wenqi W et al.

Intraperitoneal metastasis is a frequent progression in colon cancer, highlighting the critical need for enhanced therapeutic approaches. Oncolytic virus (OV) therapy has emerged as a promising immunotherapeutic strategy. The combination of OVs with immune checkpoint inhibitors (ICIs) has demonstrated an enhanced anti-tumor response. We generated the intraperitoneal-subcutaneous bilateral colon cancer xenograft model to simulate intraperitoneal metastatic colon cancer, and evaluated the systemic antitumor efficacy of the oncolytic HSV-1 virus VT1092M in combination with ICIs (anti-PD-L1 antibody and anti-CTLA-4 antibody). Intraperitoneal administration of VT1092M markedly suppressed intraperitoneal cancer progression and enhanced the superior antitumor efficacy of ICIs immunotherapy in murine intraperitoneal metastatic colon cancer. Notably, treatment of VT1092M combined with anti-CTLA-4 antibody could eliminate both intraperitoneal metastases and subcutaneous tumors, while also inhibiting the formation of peritoneal blood vessels and malignant ascites. The synergistic combination of VT1092M and anti-CTLA-4 elicited potent intraperitoneal and systemic antitumor immunity, culminating in durable, tumor-specific memory that significantly prolonged survival in mice with colorectal intraperitoneal metastases compared to either monotherapy. Mechanistically, the combination therapy activates the anti-tumor immune environment within the peritoneal cavity, leading to a significant increase in tumoricidal CD8+ T cells and M1 macrophages, as well as the reprogramming of transcriptional signatures associated with the tumor immune response within the tumor microenvironment. Ultimately, intraperitoneal immunotherapy with VT1092M enhances the efficacy of immune checkpoint blockade in suppressing intraperitoneal metastatic colorectal cancer and induces durable tumor-specific immune memory, thereby exerting substantial anti-tumor effects. This finding highlights the potential of synergistically combining oncolytic viruses with ICIs to advance immunotherapeutic strategies for intraperitoneal metastatic colorectal cancer.

Histopathologic Assessment of Pulmonary Capillaritis Using Combined Immunohistochemical and Special Stains: An Autopsy Case Double-Positive for MPO-ANCA and Anti-GBM Antibodies.

Watanabe Mari M, Hasegawa Shun S, Miyauchi Yoshihiro Y, Matsubara Daisuke D et al.

Anti-glomerular basement membrane (anti-GBM) disease may coexist with antineutrophil cytoplasmic antibodies (ANCA), a clinicopathologic entity termed double-positive disease, in which diffuse alveolar hemorrhage (DAH) is common but histologic confirmation of pulmonary capillaritis remains difficult. A 93-year-old woman presented with rapidly progressive renal failure, and serologic evaluation revealed markedly elevated myeloperoxidase (MPO)-ANCA and anti-GBM antibody levels. Despite corticosteroid therapy, the patient died of progressive renal failure. At autopsy, the kidneys demonstrated diffuse crescentic glomerulonephritis with linear IgG deposition along the glomerular basement membrane, consistent with anti-GBM nephritis, together with multiple cortical scars composed of clusters of globally sclerotic glomeruli and interstitial fibrosis. The lungs exhibited DAH with focal neutrophilic infiltration along the alveolar septa, accompanied by focal destruction of capillary walls with subtle leukocytoclasia. Immunohistochemical analysis using epithelial marker (cytokeratin AE1/AE3), endothelial markers (CD31, CD34, and ERG), and platelet markers (CD61 and CD42b) highlighted alveolar epithelial injury, loss of endothelial continuity, and extravascular platelet deposition, respectively, in areas where capillaritis was suspected. Spatial concordance of these findings supported the presence of structural injury associated with pulmonary capillaritis. This case highlights the diagnostic importance of rigorous histopathologic assessment complemented by combined immunohistochemistry in patients with DAH.

Evaluation of the anti-inflammatory potentials of ethanol leaf extract of Vigna unguiculata in Wistar rats.

Adegbuyi Adekunle Theophilus AT, Omoirri Aziakpono Moses AM, Joseph Oyepata Semeon OS, Olanlokun John Oludele JO et al.

Inflammation involves complex biological pathways, notably characterized by the overexpression of cytokines such as IL-1β, TNF-α, IL-6, IL-8, and COX-2. Inflammation is the onset of many diseases and its control will prevent several progression and complications of many diseases. This study investigates the anti-inflammatory activity of ethanol leaf extract of Vigna unguiculata in rat models, with emphasis on its effects on serum inflammatory cytokines and acute-phase reactants. Ethanol extract of Vigna unguiculata leaves was prepared and evaluated for anti-inflammatory activity using carrageenan-induced paw edema and egg albumin-induced paw inflammation models in Wistar rats. Serum levels of IL-1β, TNF-α, IL-6, IL-8, IL-10, IL-13, IFN-γ, CRP, and COX-2 were quantified using ELISA after treatment with extract doses of 100, 200, and 400 mg/kg. Indomethacin (10 mg/kg) served as the standard control. Phytochemical screening and acute toxicity testing were performed. The extract significantly inhibited paw edema in a dose-dependent manner, with 400 mg/kg showing a 63.2% inhibition at 4 h post-carrageenan injection. Biomarker analysis showed significant decrease of pro-inflammatory cytokines (IL-1β, TNF-α, IL-6, IL-8, IFN-γ, CRP, and COX-2) and increase of anti-inflammatory cytokines (IL-10 and IL-13) (p < 0.05 vs. control). No signs of toxicity were observed up to 2000 mg/kg. The anti-inflammatory activity of Vigna unguiculata may be attributed to its modulatory effects on cytokines and acute-phase reactants. This validates its traditional use and suggests potential therapeutic benefits in inflammatory diseases.
