Associations between parental generative artificial intelligence health-advice use and child vaccination uptake in Japan: an exploratory cross-sectional study.

Imanishi Yousuke Y, Iorini Maria M, Jwa Sinchul S, Sakamoto Masahiko M et al.

Parents increasingly use generative artificial intelligence (AI) for health information, but evidence on parental AI use and childhood vaccination is limited. We examined exploratory associations between regular parental AI health-advice use and childhood vaccination in Japan. This cross-sectional study analyzed 2025 nationwide survey data from children aged 1 to 17 years and parents, with a linked 2023-2025 sub-analysis. The exposure was regular parental use of generative AI for health advice at least a few times per month. Outcomes were age-appropriate measles-rubella (MR) vaccination and any mumps, influenza, or coronavirus disease 2019 (COVID-19) vaccination. Modified Poisson regression estimated adjusted prevalence ratios (aPRs) and 95% confidence interval (95% CI) with parent-level clustering and covariates. Among 8282 children and 5170 parents, 1278 (15.4%) had regular AI health-advice users as parents. AI use was not associated with age-appropriate MR vaccination (aPR 1.02; 95% CI 0.97-1.09), but was associated with three vaccines. The adjusted rates were 76.6 versus 72.7% (aPR 1.05, 95% CI 1.01-1.10) for mumps, 56.7% versus 49.0% (aPR 1.16, 95% CI 1.08-1.24) for influenza, 40.6% versus 30.9% (aPR 1.31, 95% CI 1.19-1.45) for COVID-19. The COVID-19 association was strongest among parents younger than 30 years (aPR 2.58; 95% CI 1.90-3.50; P for interaction < 0.001). In the linked sub-analysis, these associations persisted after adjustment for an available 2023 household-level prior vaccination marker and attitudes. Regular parental use of generative AI for health advice was associated with mumps, influenza, and COVID-19 vaccination, but not with age-appropriate MR vaccination. These exploratory, cross-sectional findings are observational associations. They may reflect digital health engagement, health information seeking, socioeconomic differences, or favorable attitudes, rather than vaccine-specific AI advice.

Functional Mitral Regurgitation Trajectories in CRT-Treated HFrEF During Medical Therapy Optimization: Association with Reverse Remodeling.

Pătru Oana O, Luca Silvia S, Cozma Dragoș D, Enache Bogdan B et al.

Background: Functional mitral regurgitation (MR) is common in patients with heart failure with reduced ejection fraction (HFrEF) and may persist despite cardiac resynchronization therapy (CRT). Sacubitril/valsartan is associated with reverse remodeling in HFrEF, but MR dynamics in CRT-treated patients receiving sacubitril/valsartan remain insufficiently characterized. We aimed to characterize longitudinal changes in MR severity and assess their association with ventricular reverse remodeling in CRT-treated patients with HFrEF receiving sacubitril/valsartan. Methods: This retrospective cohort study included 188 patients with HFrEF previously treated with CRT who subsequently initiated sacubitril/valsartan. Clinical and echocardiographic assessments were performed at baseline and 10-14 months. The primary endpoint was MR improvement, defined a priori as a reduction of at least one MR grade. Changes in paired MR grade distribution were assessed using Bowker's test of symmetry. Associations between MR improvement and changes in left ventricular ejection fraction (LVEF), left ventricular end-diastolic volume (LVEDV), and left atrial volume (LAV) were evaluated. Multivariable logistic regression was used to identify factors independently associated with MR improvement. Results: MR improvement occurred in 116 patients (62%), while 72 (38%) had unchanged grades; no worsening was documented in the paired assessments. A significant shift toward lower MR grades was observed between baseline and follow-up (χ2 = 116.0, df = 5, p < 0.001). The proportion meeting the endpoint increased from 0% in baseline grade I to 51.2%, 80.0%, and 100% in grades II, III, and IV, respectively. Patients with MR improvement had greater increases in LVEF (9.0 vs. 7.0 percentage points, p = 0.006) and larger reductions in LVEDV (-45.7 vs. -28.6 mL, p = 0.004) and LAV (-20.4 vs. -4.2 mL, p < 0.001). The correlation between changes in LVEDV and MR grade was statistically significant but modest (r = 0.22, p = 0.002). Non-ischemic etiology and greater baseline LVEDV were independently associated with MR improvement, whereas baseline LVEF was not. The strong association with baseline MR grade partly reflects the endpoint's inherent dependence on initial severity. Conclusions: In patients with HFrEF treated with CRT who also received sacubitril/valsartan, lower MR grades were observed at follow-up and were associated with greater longitudinal changes in LVEF, LVEDV, and LAV. These findings describe parallel longitudinal changes in MR severity and cardiac structure within a combined device- and pharmacological-treatment setting. The observed changes cannot be disentangled from ongoing CRT-mediated remodeling, concomitant GDMT, changes in loading conditions, or other time-dependent factors and cannot be attributed specifically to sacubitril/valsartan.

Vascular Plug Closure of Delayed Leaflet Perforation Post-TEER in Patient With Multiple Implantable Devices.

Chen Guanzhong G, Xiong Zhenyu Z, Liu Menghui M, Zhou Ziwei Z et al.

Transcatheter edge-to-edge repair (TEER) is the main minimally invasive therapy for severe mitral regurgitation (MR) in patients at high surgical risk. Roughly 10% of patients develop recurrent MR within 1 year postoperatively. Postprocedural mitral anatomical distortion complicates reintervention, rendering recurrent MR management a common clinical challenge. A 63-year-old man with multivalvular heart disease, chronic heart failure, multiple implantable devices and dialysis-dependent end-stage renal disease presented with symptomatic severe recurrent MR secondary to delayed anterior mitral leaflet perforation after TEER. Under combined fluoroscopic and 3-dimensional transesophageal echocardiographic guidance, our multidisciplinary team performed transcatheter closure of the perforation using a vascular plug. MR was markedly alleviated, heart failure symptoms resolved, and the patient recovered uneventfully before discharge. TEER-related leaflet perforation caused severe recurrent MR. Percutaneous plug closure successfully downgraded MR to trivial, confirming the feasibility of this approach for mitral leaflet perforation.

Optimizing Echocardiographic Evaluation of Chronic Mitral Regurgitation: Comparison With Cardiac Magnetic Resonance Imaging and Implications for Guideline Recommendations.

El-Tallawi K Carlos KC, Angulo Clara C, Chamsi-Pasha Mohammed M, Qamar Fatima F et al.

The ASE (American Society of Echocardiography) guidelines recommend the presence of at least 4 specific criteria to confirm a diagnosis of definite severe mitral regurgitation (MR). However, the performance of these criteria, particularly when <4 are present, remains unclear. This study aims to evaluate and optimize the ASE native MR guidelines by comparing them with quantitative cardiac magnetic resonance (CMR), considered the reference standard for MR assessment. In this prospective study, 177 patients underwent near-simultaneous CMR and transthoracic echocardiography for the assessment of MR. MR severity was classified using a 4-grade scale based on CMR-derived regurgitant volume, which was calculated as the difference between total left ventricular (LV) stroke volume and aortic forward flow. MR severity was similarly graded using the ASE algorithm, with quantification performed when necessary, by averaging the regurgitant volume from 3 methods: 1) the proximal isovelocity surface area method; 2) the biplane LV volumetric method; and 3) the mitral valve volumetric method. The study cohort had a median age of 61.7 years, with 55% being men. Primary MR was the predominant pathology (73% of patients), with 42% having mitral valve prolapse and 25% presenting with flail leaflets. There was exact agreement between transthoracic echocardiography and CMR MR grading in 51% of cases, whereas agreement within 1 grade was observed in 89%. Patients who met any of the ASE severe MR criteria were predominantly classified as having grades 3 to 4 MR by CMR. The following ASE parameters demonstrated high specificity for identifying grade 4 MR by CMR: pulmonary vein systolic flow reversal (specificity 88%), enlarged LV with normal systolic function (specificity 85%), proximal isovelocity surface area radius ≥1.0 cm (specificity 83%), and vena contracta width ≥0.7 cm (specificity 80%). Combining color Doppler with either pulmonary vein systolic reversal or enlarged LV with normal function resulted in 91%-95% specificity for severe MR. Importantly, reliance on fewer ASE criteria reduced the need for quantification from 86% (4 criteria) to 73% (3 criteria) and 59% (2 criteria), thereby enhancing diagnostic efficiency. Certain ASE criteria, especially when used in combination, are highly specific for diagnosing severe MR, even when <4 criteria are present. This study suggests that in many cases, the reliance on extensive quantification may be reduced, streamlining the diagnostic process and improving clinical workflow.

Mixed Reality-Based Monitoring and Supervisory Control of an Industrial Assembly Line Using Direct OPC UA and Modbus TCP Communication.

Covrig Tudor T, Duka Adrian A, Miclea Liviu L

Supervisory monitoring and control systems require prompt access to process data and information. The systems described in the literature utilize Mixed Reality (MR) technology and have different system architectures and communication methods. Some systems require the presence of intermediary elements for data exchange between the MR environment and the industrial control system. In this article, we suggest and develop an MR-based system for the supervisory monitoring and control of an industrial assembly line. The uniqueness of the suggested system consists of bidirectional communication between the MR system and the main PLC using the OPC UA protocol without intermediary elements in the MR-PLC communication chain. The Modbus TCP protocol is used for obtaining parameters from the network analyzer. The system allows us to monitor some process parameters and sensors on the assembly line and transfer supervisory commands from the MR interface to the main PLC using virtual controls on physical elements. The system was built using the MRTK library inside the Unity environment. To experimentally evaluate the system, experiments were conducted on the industrial assembly line, where the process parameters and communication performance were recorded under experimental conditions.

[Surgical treatment of anterior visual pathway gliomas in children using intraoperative neurophysiological monitoring and MR tractography].

Golovin N I NI, Gorelyshev S K SK, Ogurtsova A A AA, Afandiev R M RM

Surgical resection of anterior visual pathway gliomas carries a high risk of postoperative visual deficit. The goals of surgical treatment are maximum safe removal of tumor and histological verification. Diffusion tensor imaging (DTI) with reconstruction of visual pathways and intraoperative monitoring of visual evoked potentials (VEP) were proposed to reduce the risk of potential visual dysfunction. To demonstrate two cases of surgical treatment for anterior visual pathway gliomas using preoperative MR tractography of visual pathways and intraoperative VEP monitoring. Two patients with anterior visual pathway tumors underwent preoperative MR tractography of visual pathways. Intraoperative VEP monitoring was performed. Extent of resection in both cases was determined by preoperative MR tractography and intraoperative neurophysiological monitoring of VEP. Ophthalmological status in early postoperative period demonstrated positive dynamics in the 1st patient and no changes in the 2nd patient. Preoperative MR tractography of visual pathways and intraoperative neurophysiological monitoring of VEP with electrophysiological identification of visual pathways may potentially enable safer tumor resection and minimize the risk of postoperative visual deficit.
