[Effects of uroacitides on the methylation of PTEN gene in myelodysplastic syndrome cells and its mechanism].

To investigate the methylation patterns of PTEN gene in myelodysplastic syndrome (MDS) cell line MUTZ-1 cells and the primary cells, to explore the effects of uroacitides (CDA- II) on the methylation patterns, and to provide novel target therapy for MDS. Reverse transcriptase-polymerase chain reaction (RT-PCR) and Western blot were used to determine the expression of PTEN and DNA methyltransferases (DNMTs) as well as the effects of CDA-II on them. Methylation specific PCR (MSP) was used to identify the methylation patterns of PTEN and the effects of CDA-II on them. PTEN was completely methylated in high-risk MDS and acute myeloid leukemia (AML) transformed from MDS. High expressions of DNMTs proteins, especially DNMT3b protein were found in high-risk MDS and AML cells transformed from MDS. PTEN expression level was increased from 0.02 ± 0.01 at control group to 0.23 ± 0.11, 0.54 ± 0.11 and 0.92 ± 0.13 after treatment with CDA-II at 2, 4 and 8 mg/ml (P<0.01). If the cells were treated with CDA-II at 4 mg/ml for 12, 24 and 48 hours, PTEN expression level was up- regulated to 0.15 ± 0.06, 0.52 ± 0.12 and 0.89 ± 0.13, which were significantly higher than that of control group 0.02 ± 0.01 (P<0.01). CDA-II could significantly down-regulate the expression of DNMTs in MUTZ-1 cells in a dose-(r=0.999, 0.992, 0.995, P<0.01) and time-dependent (r=0.989, 0.981, 0.985, P<0.05) manner, which led to the demethylation of PTEN. Among them, DNMT1 was the most downregulated (F=21.256, P<0.05). PTEN gene was hypermethylated in high-risk MDS, and target therapy of PTEN demethylation might be a new strategy for MDS therapy. CDA-II could down regulate the expression of DNMTs and lead to PTEN demethylation which induce MUTZ-1 cells apoptosis.

[Multicenter phase II clinical trial of uroacitides injection in the treatment for advanced malignant tumors].

To investigate the efficacy, safety and the life quality improvement of uroacitides injection in the treatment for patients with advanced malignant tumors. A total of 160 patients with advanced stage cancers were enrolled into this multicenter, open and non-randomized phase II clinical trial, including cancers of the lung (33 cases), liver (45 cases), breast (17 cases), esophagus (11 cases), stomach (18 cases), colon (19 cases), pancreas (3 cases) and kidney (4 cases), and glioma (10 cases). Uroacitides was administrated in a dose of 300 ml daily via the superior vena cava catheter for consecutive 4-8 weeks. Of the 160 patients, 21 dropped out and one patient died during the trial. Efficacy could be evaluated in 138 patients and safety in 160. The total objective response rate (ORR, CR + PR)) and tumor control rate (CR + PR + MR + SD) of the 138 evaluable patients were 5.8% and 65.2%, respectively. Clinical benefit response (CBR) rate was 57.2%. Major adverse effects were grade I - II and reversible nausea/vomiting (21.9%) and pain (6.3%). Uroacitides injection is effective in the control for various kinds of advanced cancers with mild, reversible and tolerable adverse effects, and can also improve the patient's quality of life. It is worth being studied further.

[A randomized clinical trial of Uroacitides combined with NP and NP regimen alone for advanced non-small cell lung cancer].

Uroacitides is a group of cell differentiation inducers, which is purified from fresh human urine. Preclinical studies of Uroacitides have showed that cancer cells could be induced to differentiate, and the growth of cancer cells could be inhibited by Uroacitides. The aim of this study is to compare the efficacy and toxicity between Uroacitides combined with NP regimen and NP alone in treatment of advanced non-small cell lung cancer (NSCLC). Forty-two cases of advanced NSCLC were randomized into Uroacitides+NP and NP groups. NP group: NVB 25mg/m² on days 1 and 8, DDP 75mg/m² on day 1. Uroacitides combined with NP group: Uroacitides of 300mL was given through subclavian catheter daily for 7 days prior to the NP chemotherapy, then concurrently with NP regimen for 2 cycles, except the days of administration of chemotherapy. In the Uroacitides+NP group, the overall response rate was 44.4%, and 20.0% in the NP group (P > 0.05). The median survival time was 9 months in the Uroacitides+NP group and 6 months in the NP group (P=0.0287). The main toxicities were myelosuppression, gastrointestinal reaction and alopecia, and there was no significant difference in incidences of toxicities between the two groups (P > 0.05). Uroacitides combined with NP regimen shows a good curative effect and low toxicity, and may significantly prolong the median survival time for advanced NSCLC.
